Hero Family Outreach (HFO) requires certain forms/letters to qualify for consideration for a service dog under our Paws 4PTSD program.

 

Please have the following available when completing the following application:

1. Proof of medical diagnosis of service-related PTSD by a licensed physician

2. Proof of continual counseling services for at least 1 month prior to application

3. DD214 containing Honorable or Medical Discharge and Combat Service (Military) or Retirement/Medical Retirement form (Law Enforcement/Fire Service)

4. Copy of your Photo ID

After your application is reviewed and preliminarily approved, the next step in qualification is a phone interview with the HFO board of directors and in-home assessment to successfully pair you with the best service dog that will compliment your personality and your family members, including other pets in the home.

You will receive notification by phone if all pre-qualifications have been met and you have final approval to become a recipient of a Paws 4PTSD Service Dog. The complete process from application, and training of the dog, to receiving a dog may take up to 6 months as a PTSD dog is trained for the specific needs of the recipient.

All information received is confidential and will only be shared with Hero Family Outreach Board Members for the purpose of determining eligibility for the Paws 4PTSD program. HFO asks that you black out social security numbers before attaching documents.
